Qualifications

About The Role

Are you an experienced Advanced Clinical or Nurse Practitioner, looking for the next step in your career?

This exciting, new Clinical Lead position within the Unscheduled Care Coordination Hub could be for you.

The UCCH is an award-winning innovative multi-disciplinary hub aimed at facilitating improved patient outcomes, admission avoidance and enhanced patient journeys. Clinicians and paramedics from Integrated Urgent Care, East of England Ambulance Trust and the Urgent Community Response teams work together to proactively intercept category 3-5 ambulances in an effort to reduce demand on the ambulance trust and emergency department.

By working alongside other providers in primary care, secondary care, mental health, palliative care, community and voluntary organisations, the UCCH aims to manage patients within the community, keeping care closer to home.

The 20 hours Part-Time position of UCCH Clinical Lead is a dual managerial and clinical role upholding the delivery of a quality and safe service for patients accessing urgent care services within Suffolk and North East Essex. This role is required to be Face to Face and cannot support remote working.

The role requires you to practice autonomously as an Advanced Clinician and support the local IUC medical lead and service manager (Registered CQC Manager) to ensure high quality performance and governance of the UCCH.

What you'll be doing

Main Duties

  • 8 management hours and 12 clinical hours per week to be worked within the UCCH
  • Promote and support the UCCH clinical audits and undertake quality assurance and governance activities.
  • Have an active role in recruitment, training and development to help promote and deliver a safe and efficient service.
  • Work with external stakeholders including commissioners and Quality leads
  • Clinicians working in the UCCH will be trained to work in the Clinical Assessment Service (CAS) and have the option of working a hybrid role between the UCCH and CAS and/or North East Essex OOH services.
Management:

  • Provide direct line management, annual appraisal and clinical supervision for the clinical team
  • Promote educational activities, development and evidenced based practice within the workforce
  • Undertake and fully participate in the UCCH audit process and clinical governance activities
  • Support the medical lead and quality team in the investigation of feedback, complaints and incidents
  • Work with external stakeholders including commissioners and quality leads
  • Represent the UCCH at external meetings
Clinical:

  • Working face to face as part of the multi-disciplinary UCCH team and practise autonomously within sphere of competency
  • Reviewing low acuity cases held in the East of England Ambulance Service (EEAST) queue and identifying cases that may be suitable for coordinated community-based care
  • Completing remote consultations via telephone or video
What we'll look for in you

Essential

  • ANP/ACP/UCP educated to masters (or equivalent) level and with non-medical prescribing qualification
  • Full MSc or sufficient PHC experience with evidence of supporting CPD at level 7 study.
  • At least 6 months experience of independent consulting and prescribing within urgent, primary or community care
  • Ability to consult independently for all ages, including under 2 years old
  • Understanding of integrated urgent care services including NHS111, CAS and OOH
  • Knowledge of how other system partners and health and social care services operate including (but not exclusive to): primary care, urgent treatment centres, emergency department, pharmacies, ambulance service
  • Hold full registration and professional indemnity (where appropriate).
  • Desire to provide holistic care for patients and interest in promoting patient safety and improving patient outcomes
Desirable

  • Current or recent UCCH/CAS/OOH experience
  • PaCCS trained
  • Experience of remote patient consultation
