Are you an outstanding Clinical Lead looking for new challenges in your career? We are looking for a Multi-Professional Clinical Lead to join our new Specialist Community Learning Disability Service. This is an exciting opportunity to join the team as we introduce a new clinical model, offering standard and enhanced pathways provided in community settings, close to people's homes. Your expertise will be the driving force for change to ensure high-quality care, delivery of health and well-being outcomes for adults with learning disabilities and their families.
The Clinical Lead will be a visible member of the SCLDS Leadership Team, proactively supporting the multi-disciplinary SCLDS to achieve high standards of clinical care delivery across multiple professional boundaries. You will lead and present complex, sensitive, and challenging clinical and professional communication processes that may impact on service users, carers, staff, the team, the organisation, partner agencies, and members of the public, where there are barriers to understanding.
The Clinical Lead post is a pivotal position to drive forward the ambitions of the Learning Disabilities transformation programme, developing and delivering on good quality, multi-disciplinary care. Responsibilities include timely access to services, positive experience and outcomes, the delivery of equitable person-centred care, and actively working to reduce health inequalities for those we support.
You will be a visible member of the SCLDS Leadership Team, proactively supporting the multi-disciplinary SCLDS to achieve high standards of clinical care delivery across multiple professional boundaries. You will be responsible for Multi-professional Quality, Safety, Clinical Risk, Strategy, Transformation and Planning.
The Clinical Lead will work with Clinical Directors, Senior Managers, and Professional Leads to develop and implement the clinical strategic & transformational developments in SCLDS and, with other colleagues, more broadly in SHSC and the wider system.
The Clinical Lead will work closely with the Senior Responsible Officer leading improvement work across South Yorkshire on Stopping over-medication of People with Learning Disability (STOMP) and The Learning Disability Mortality Review (LeDeR).
At SHSC, we prioritise the well-being and safety of both our service users and employees. It is our policy and a condition of employment that all employees must join the DBS Update Service. This service carries an annual fee, which employees are responsible for paying up front. However, employees may claim this cost back through the Trust's Employee Expenses Reimbursement Policy. For those requiring a Basic DBS check, it is necessary to register for a DBS Online Account.
