Job summary

HCRG Care Group is recruiting a Clinical Lead Nurse to head the Intermediate Care and Reablement Service in Medway. You will lead nursing teams, ensure evidence-based, person-centred care, and support capacity planning across community and bedded pathways.

You’ll drive workforce development, clinical governance and service improvement, working with senior colleagues to shape care delivery and patient outcomes in the Medway region.

Qualifications

• Evidence of leadership or management training and experience in community or intermediate care.
• Experience with clinical governance, safeguarding and quality improvement initiatives.
• Strong communication and relationship-building abilities for multidisciplinary teams.

Responsibilities

• Provide visible clinical leadership across Intermediate Care and Reablement Service.
• Support safe discharge planning and manage patient flow and capacity.
• Contribute to workforce development and quality improvement activities.
• Collaborate with senior therapy colleagues and multidisciplinary partners.
