A healthcare organization in Sussex is seeking a Senior Clinical Lead to provide high-quality care and support to patients in their homes. The role includes leadership responsibilities, staff management, and collaboration with various healthcare partners. Ideal candidates will have a registered healthcare qualification and strong clinical assessment skills, promoting recovery and independence for patients. Flexible working options and comprehensive training programs are offered.
Are you looking for a role where every day brings new challenges and the opportunity to change the lives of people in Sussex? Now is the perfect time to be part of our evolving Home First Service.
We’re bringing together nurses, therapists, health and therapy assistants to work alongside acute multi-disciplinary teams and transfer of care hubs to promote an efficient, effective hospital-to-home transition for our patients.
Our multi-disciplinary Home First teams work flexibly to provide a 7-day service, 365 days a year, between 8am and 8pm.
Patients discharged from hospital into Home First will receive step-down intermediate care, involving community-based assessments and interventions provided in their own homes to support recovery and monitor their health post-discharge to prevent re-admission.
Home First will, where required, provide acute reablement for up to 10 days, supporting people to achieve functional goals and work towards independence with activities of daily living.
This is more than just a job—it’s a chance to be part of an innovative wrap-around recovery service that helps people recover at home and regain their independence, giving them the confidence to return to daily life.
The focus of this role is providing safe, effective, and high-quality senior clinical care, along with strong clinical leadership, advice, and support to the team.
You will use your comprehensive knowledge and champion excellence in practice in a dynamic healthcare environment. Your specialist clinical assessment skills will support patients with complex needs, in addition to providing clinical advice and support to colleagues.
Working with operational leads, you will ensure the clinical effectiveness of the service and the delivery of an excellent patient experience. You will also contribute to service development through reflective practice, clinical supervision, and quality improvement initiatives. Supporting interdisciplinary working and cross-skilling for all staff, fostering a collaborative learning environment, is central to this role.
Your experience working across various clinical settings and autonomously will help you excel. We will support your development in specialist practice areas and leadership skills through internal and external programmes.
In this role, you will be responsible for staff management, supervision, and development, including shift coordination, allocation of visits, and conducting performance reviews.
The team collaborates closely with Sussex Community NHS Foundation Trust services, acute and community partners, GPs, SECAmb, adult social care, and voluntary and third sector organizations.
You will need to be able to drive with a full UK license and have access to a vehicle due to the geographic area covered by the team.
