Role Details
The LWBC post supports clinical teams in delivering the LWBC agenda, which is key to providing future follow-up care for patients living with and beyond cancer.
As survivorship trends increase, pathways will include stratified follow-up, complex case management, remote monitoring, patient-initiated follow-up, personalized care plans, and treatment summaries. This role offers an opportunity to improve patient experience, increase capacity, and optimize resource use.
The post holder will be a member of the Cancer Services team, supporting this agenda.
37.5 hours over 4 or 5 days
The advertised salary reflects the Agenda for Change 2025/6 rates, including a 3.6% pay award to be implemented later this year. If you join before then, the pay award will be backdated to your start date. Until then, remuneration is based on 2024/25 pay bandings (Pay scales for 2024/25).
