City and Hackney is advancing a bold neighbourhood-basedmodel of careanchored in Primary Care Networks (PCNs), shaped by communities,and aligned with wider system reform across North East London. We are seeking asenior clinical leader with a strong track record in system transformation toguide the development of our eight integrated neighbourhoods.
This is a high-impact strategic leadership role offering theopportunity to influence and embed a maturing neighbourhood infrastructure thatsits at the intersection of place-based partnerships, PCNs, and integrated carepriorities.
As Clinical Lead for Integrated Neighbourhoods, you willprovide strategic clinical leadership to ensure that neighbourhood activity isboth responsive to local needs and aligned with system-wide objectives. Youwill work across organisational and professional boundaries to reframe localdelivery models, embed a shared population health approach, and strengthen theenabling role neighbourhoods can play in supporting PCN and Place ambitions.
You will also play a key role in leadership development,supporting the growth of a confident, skilled, and inclusive workforce to carrythe model forward.
All 37 GP practices in City & Hackney are members of City & Hackney Integrated Primary Care with practices covering a growing registered patient list size of nearly 353,000
What were looking for:
Familiarity with City and Hackney or comparable boroughs is desirable, as is experience using PHM tools and leading integrated initiatives.
