JOB TITLE: Health Inequalities Long Term Conditions (HILTC) Team Clinical Lead

RESPONSIBLE TO: Associate Medical Director / Medical Director

ACCOUNTABLE TO: Director of Operations

HOURS OF WORK: 1-2 sessions per week (Mondays)

REMUNERATION: £9,500 - £10,500 per session per annum dependent on experience

CONTRACT LENGTH:Fixed Term 1 year Maternity Cover

MAIN LOCATION: Haringey GP Federation, 48 Station Road, Wood Green, London, N22 7TY

Main duties of the job

The serviceproactively case finds patients in the east of the borough, who are at higherrisk of long-term conditions due to wider socioeconomic determinants of health,also known as health inequalities. These patients are invited for a relevant healthcheck with the aim of early detection, diagnosis and management of Hypertension(HTN), chronic kidney disease (CKD) and Chronic Obstructive Pulmonary Disease(COPD).Patients also are considered holistically and have theopportunity to work through further non-clinical issues such as housing andemployment with our wellbeing advocates.

To provide clinical leadership within theservice to support successful delivery and management.

Provide clinical quality assurance ofmaterials, policies and procedures for service delivery.

Supporting team members with scheduled 121s andpersonal development.

Support recruitment needs for clinicians withinthe service.

To provide clinical support and guidance toteam members attending MDTs which could include starting patients onmedication.

To represent the federation within thecommunity advocating for health equality. This might include attendance atcommunity events, or representing the service at Federation, neighbourhood,PCN, or practice level meetings.

To lead on clinical safety and governance forthe service.

To support investigation of significant events,complains, or concerns within the service in a timely manner followingprotocol.

Complete regular clinical audits for thepurposes of maintaining clinical safety, best practice, or for QI purposes.

Support improvement in patient outcomes byreviewing feedback, audits, and internal reviews.

Supporting the wellbeing advocacy team withoutreach and events including supporting with relevant clinical guidance,experts, and more.
