Clinical Lead GP - Team Up, Derby City South PCN

We are seeking an experienced GP to support our new home visiting service for housebound patients, focusing on delivering care in the most appropriate and efficient manner.

As an employee of DCHS NHS FT, you will provide care for patients within Derby City South PCN, collaborating closely with GPs, PCN staff, the HVS Team, and wider DCHS Community Teams.

Main duties of the job
  • Triaging and managing referrals to the HVS and assigning them to appropriate clinicians.
  • Providing support, advice, and guidance to the team.
  • Offering clinical leadership for the multi-disciplinary and multi-agency service to ensure high-quality primary care for housebound patients.
  • Mentoring, guiding, and supporting clinical decision-making across the team.
  • Conducting medical assessments and management in collaboration with team members.
  • Performing senior clinical triage for the Acute Home Visiting service to determine response urgency based on clinical need.
  • Providing education and mentorship, especially to advanced or extended practice staff and trainees.
  • Participating in quality improvement activities, including audits, service improvements, PDSA cycles, and learning events.
  • Building relationships with social care, ambulance services, social prescribers, mental health, and community clinicians.

This is a new team of skilled healthcare professionals employed by DCHS with full NHS terms, conditions, and access to the NHS Pension scheme. You will work on behalf of Derby City South PCN.

Our purpose is to deliver community health services to over one million residents in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ approximately 4,200 staff caring for patients across hospitals, health centres, clinics, GP practices, schools, care homes, and in patients' homes or via virtual consultations.

During the pandemic, we led public vaccination efforts in collaboration with health and social care partners.

You will work with a small team of Community GPs supporting a multidisciplinary team in Derby City South PCN.

This involves direct patient work, either in person or remotely, and contributing to the development of Team Up and the HVS in collaboration with other clinicians.

You will work with PCN leadership, Home Visiting Service clinical leads, and Community GPs to develop and deliver safe, effective services, including new clinical pathways aligned with local and national standards, covering areas such as Acute Home Visiting, urgent community response, care in care homes, and anticipatory care.

You will engage with local stakeholders and partners to promote a multi-agency approach to care delivery for local residents.

Person Specification
Education, Qualifications, and Training
  • Doctor/Consultant registered with the relevant Specialty Register
  • Current GMC registration
  • Royal College membership
  • Mentorship and Supervision qualification or equivalent experience (e.g., GP Trainer, DMP)
  • Qualification or experience in teaching programmes
Experience and Knowledge
  • Interest in and knowledge of community care
  • Experience working in multidisciplinary teams
  • Experience with frailty in patients or willingness to develop skills in this area with support
  • Experience in medical education and clinical supervision
  • Ability to lead across organizational and professional boundaries

Salary: Depending on experience, £12,500 per session.
