Job summary

Midlands Partnership NHS Foundation Trust is seeking a Clinical Lead to supervise the Forensic Transitions Team as adults move from secure settings to community care. The role requires a first-level professional qualification in nursing, AHP or social work, with post-registration experience in forensic settings.

You will lead assessments, ensure performance against KPIs and budgets, and drive service development across multidisciplinary teams.

Qualifications

• First-level professional qualification in nursing, AHP or social work
• Post-registration experience or formal training at postgraduate diploma level
• Current registration as a mental health nurse, social worker or AHP
• Extensive post-registration experience in forensic settings with adults who have mental health needs or learning disabilities
• Experience integrating care delivery across multidisciplinary and multi-agency teams
• Experience leading or contributing to multidisciplinary assessment processes
• Experience carrying out clinical audit
• Knowledge of health and social care legislation, Care Programme Approach principles and related standards

Responsibilities

• Provide day-to-day clinical leadership and oversee delivery of the Forensic Transitions Team’s services
• Lead specialist assessment, formulation and treatment during transition to community services
• Ensure performance against trust standards, KPIs and financial targets
• Monitor and manage team performance against trust objectives
• Provide advice to team staff, multidisciplinary teams and management groups
• Monitor and evaluate practice against current trends and initiate changes where needed
• Ensure statutory and professional responsibilities under the NMC Code and other bodies are met
• Promote integration between community and inpatient services
• Encourage multidisciplinary and multi-agency working
• Build strong working relationships with internal and external services
