An exciting opportunity has become available in North Cumbria Memory and Later Life Service for a Band 7, Clinical Lead with a focus on our collaborative working with Primary Care.
The Primary Care Clinical lead will support Memory and Later Life Service in providing effective support to Primary care and overseeing service developments in line with CNTW Transformation programme. The post will also focus on improving access to memory services, diagnosis of new or suspected dementia.
In this role you will work as part of a collective leadership team covering North Cumbria
This post offers a clinical leadership role focussing on improving close working with Primary care services and supporting transformation including standardising MLL referrals to a single point in line with the CBU’s Initial Response service. This will also include developing primary care-based screening and support to take forward Cumbria Dementia Group’s improvements in access to diagnosis
