We are looking for a Clinical Fellow to support our Inpatient team,primarily undertaking hospice admissions/follow-ups and urgent assessments ofpatients on the In-patient unit.

This post will suit a doctor who already has some experience inPalliative Medicine and may be planning to follow a career in the specialty butwishes to have additional experience prior to application for SpecialtyTraining.

You will support and provide cross cover across themedical team, assessing and managing patients admitted to the hospice, with aholistic multi-professional team approach. You will join a supportive medicalteam, have a clinical supervisor have access to CPD and teaching opportunities.

Main duties of the job

Having strong communication and people skills is fundamental as you willbe required to work in partnership with the multi-professionalteam, ensuring every patient and their needs are always at the centre ofwhat you do.

Finally, you will be able to provide evidence of full registration, witha GMC licence to practice and ongoing appraisal / revalidation.

Job responsibilities

2. Keyresponsibilities

  • To assess and manage patients admitted to the hospice,with a holistic Multi Professional Team (MPT) approach.
  • To maintain ongoing medical care for in-patients,ensuring continuity of care e.g. attend / contribute to morning MPT handover.
  • To liaise closely with consultant colleagues,maintaining regular day-to-day contact.
  • To work closely with all members of the MPT.
  • To provide planned and urgent medical reviews forpatients on the in-patient unit.
  • To support the community team nurses in discussingissues in relation to the management of patients at home when on call.
  • To provide excellent communication / handover ofclinical information by telephone, email or in writing as appropriate, ensuringthat all relevant in-house / external healthcare professionals are up to datewith clinical events e.g. letters to general practitioners after a patient hasdied or is discharged from the hospice, copying in relevant specialist teams.
  • To support / address the needs of carers / families,e.g. communicating with them as often as necessary.
  • To maintain accurate and clear, contemporaneouspatient records i.e. appropriately updated electronic patient records.
  • To attend weekly MPT inpatient meetings and consultantward rounds.
  • To appropriately manage patients after their death onthe IPU; to confirm the nurse-verification of death, to complete the necessarypaperwork (Medical Certificate of Cause of Death, Medical Examiner Referral )and to notify the coroner if necessary.
  • Coordinates with colleagues to provide medical supportto the Well-being centre.
  • To work flexibly with the medical team and to providecover for colleagues and the wider MPT as appropriate.
  • To help with induction, education and supervision ofmedical students and visiting doctors as appropriate.
  • The post holder maybe required to be part of theon-call rota across Princess Alice Hospice and St Raphaels Hospice in Cheam.This is shared 1:5 first on call rota for weekdays, weekends and bank holidays.Consultants provide 24/7 second oncall cover. Adequate handover and handback across themedical team is expected.

3. General requirement for medical staff

  • To contact one of the consultants, othersenior colleague or managers, as appropriate, if there are any clinicalconcerns.
  • To participate in audit, research or anyon-going developments within the hospice and where appropriate to develop theirown audit / research.
  • To participate in the medicaldepartments CPD programme e.g. Monday meetings and the alternating monthly,Wednesday / Thursday medical journal club as well as other educationalactivities at the Hospice.
  • To participate in teaching for the widerMPT, hospice staff / volunteers and externally where appropriate.
  • To comply with Continuing ProfessionalDevelopment (CPD) requirements as stipulated by the Royal Colleges.
  • To keep up-to-date with clinical mandatorytraining requirements.
  • To keep a portfolio of evidence, to havean annual appraisal and write a personal development plan (PDP) in line withGMC revalidation policies and procedures.
  • Tomaintain their own professional indemnity insurance to cover work in thehospice.
  • To maintain confidentiality at all timesand comply with all other aspects of the General Medical Councils Code ofProfessional Conduct / Good Medical Practice.

4. General requirement for all staff

  • Toattend any staff meetings, where appropriate, in discussion with the MedicalDirector.
  • To attend statutory training, e.g. firetraining, lifting and handling and other health and safety issues.
  • To actat all times in a manner to safeguard the interests of patients and theirfamilies.
  • To be aware of and perform according tothe Hospices values and behaviours
  • To monitor, record and assist ininvestigations of any complaints from patients, visitors and staff, reportingthese to the Medical Director.
  • To befully conversant with, and participate in, the hospices Clinical GovernancePolicy.
  • To be aware of personal and othersresponsibility as defined in the Health and Safety at Work Act and COSHHregulations.

Qualifications
  • Undergraduate:
  • MB BS or equivalent
  • Registration and defence union:
  • Full and current GMC registration and licence to practice

  • Applicants knowledge is up to date and has evidence of being fit to practice safely through appraisal / revalidation.
  • MRCGP or MRCP or equivalent or working towards
  • European Certificate of Essential Palliative Care (ECEPC)
  • Diploma in Palliative Medicine or equivalent / higher
  • MSc
Experience
  • At least 4 years full-time postgraduate training (or its equivalent gained on a part time or flexible basis.
  • Evidence of achievement of relevant competencies.
  • Clinical experience:
  • At least 6 months experience in Palliative Medicine
