Job summary

Buckinghamshire Healthcare NHS Trust is seeking a Clinical Discharge Coordinator to join our team in Buckinghamshire. The role focuses on coordinating timely patient discharges and working with multidisciplinary teams to ensure safe care transitions.

The successful candidate will have knowledge of acute and community discharge processes, the Mental Capacity Act, and Continuing Healthcare/Continuing Care criteria.

Qualifications

• Knowledge and experience of discharges from acute and community hospitals
• Understanding of the Mental Capacity Act
• Experience with Continuing Healthcare and Continuing Care Act

Responsibilities

• Coordinate discharge planning for patients requiring ongoing care after hospital discharge
• Work with multidisciplinary and multi-agency staff to ensure safe and timely discharges
• Maintain accurate written and electronic records and respect data protection
• Lead patient case conferences or best interest meetings when required
• Support audits and contribute to care standards, protocols and pathways
