Job description

Clinical Development Fellow – Hospital at Home

Full Time or Part Time – Fixed term for 12 months

Salary Scale - £45,504 – £71,550 per annum, pro rata if part time

The clinical development component, combined with supervised and directed activity, will allow for 8 hours of development per week. Four hours will be dedicated to personal and professional development, including structured departmental and directorate teaching. The remaining four hours will support departmental service development priorities.

This position is designed to complement existing services and align with the departmental strategy to improve care for frail older people within Forth Valley.

The Hospital at Home service provides short-term targeted interventions that:

  1. Offer acute, hospital-level care by multidisciplinary healthcare professionals in patients’ homes or homely settings that would otherwise require hospital admission.
  2. Assess, investigate, and treat patients at home at a level comparable to an acute hospital, following established protocols and pathways.
  3. Are delivered by a highly skilled, multidisciplinary team collaborating with primary care, health and social care partnerships, and third sector agencies.
  4. Operate 7 days a week from 0800 to 2000 across all of Forth Valley, with patients assessed within 2 hours of referral and receiving daily multidisciplinary assessments and treatments at home.
  5. Interface with existing community services to reduce unwarranted variation and deliver high-quality, cost-effective care.
