Job summary

West Hertfordshire Teaching Hospitals NHS Trust is recruiting to lead the mobilisation and ongoing development of a Care Home Dementia Support Service across South and West Hertfordshire, in partnership with the Alzheimer's Society. The role focuses on clinical leadership, education and support to care home teams.

The post reports to the Associate Director of Nursing for Medicine, works 37.5 hours per week, and offers a Band 8b salary (£68,852–£79,638) on a fixed-term 2-year contract with

Qualifications

• Degree level qualification in a relevant healthcare profession or equivalent.
• Current registration with the relevant statutory regulatory body.
• Master's level qualification or demonstrable equivalent in clinical leadership, service development and programme delivery.

Responsibilities

• Lead mobilisation and ongoing development of the Care Home Dementia Support Service across South and West Hertfordshire.
• Build partnerships across care homes, primary care, community, mental health, social care and voluntary sector.
• Provide clinical leadership, education and support to care home teams.
• Manage staff, budgets and governance with a culture of learning and improvement.
• Drive quality, governance and service evaluation using data and feedback.
