Job description

1 x Substantive position

This job description provides a general outline of responsibilities and will be reviewed regularly in conjunction with the post holder. Duties may include working at different locations within the Trust and undertaking additional tasks as requested by the line manager, including project work, internal job rotation, and cover for absences. Duties are subject to change over time and may be reviewed in the future.

Key Responsibilities
  1. Co-ordinate and evaluate collaborative, patient-centred care; gather feedback and improve plans with the multidisciplinary team (MDT).
  2. Liaise and work positively with MDT members and partners. Administer medications, monitor side-effects, and oversee service user safety.
  3. Ensure the safe custody of medicines, sharps, and clinical equipment. Report all incidents following Trust procedures.
  4. Lead and support/teach junior staff on risk assessment and management, basing decisions on research evidence, individual service user needs, social context, and clinical experience.
  5. Undertake one-to-one engagement with service users in an honest, open, and non-judgmental manner.
  6. Recognise and value service users as individuals, maintaining their respect and dignity, and ensuring care respects equality.
  7. Develop care plans reflecting service user needs, in partnership with service users, the wider MDT, and family/carers.
  8. Monitor and record clinical observations, such as physical signs and vital signs, escalating any signs of deterioration appropriately.
  9. Ensure confidentiality and security of personal information in compliance with Data Protection Act, Caldicott principles, and other relevant legislation.
  10. Follow record-keeping guidelines to ensure compliance with the Freedom of Information Act 2000.
