Job summary

Birmingham Women’s and Children’s NHS Foundation Trust in Birmingham is recruiting a Clinical Case Manager for Children in Care within the West Midlands CAMHS Provider Collaborative to lead expert case management in residential care settings. You will work in Toucan’s Integrated Complex Care team, collaborating across agencies to advocate for vulnerable children and families.

Qualifications

• Qualified RMN, RNLD or Social Worker, registered with the relevant governing body if Social Worker.

Responsibilities

• Provide expert clinical advice and case management for children and young people in residential care homes commissioned by West Midlands Local Authorities.
• Manage complex cases with autonomy and professionalism, offering guidance and support to the wider network around the child.
• Champion needs of Children and Young People in residential placements across the West Midlands.
• Work collaboratively with Local Authority social care teams to ensure therapeutic needs are identified and met.
• Lead proactive case discussions, triage referrals, and unlock access to integrated care services like IROC.
• Support the Regional Hub and Local Authorities to influence quality and performance standards for residential care providers.
• Navigate high-pressure situations with confidence, compassion, and clinical expertise.
• Travel across the West Midlands region is required.
