Job summary

Hampshire and Isle of Wight NHS Foundation Trust is recruiting a Senior Clinical Assessor/Practitioner (MHN or RMN or SWE with MH specialism) in Specialist s117 Aftercare. You will conduct comprehensive assessments, case-manage complex cases and promote Personal Health Budgets and personalisation in home and community settings.

The role requires excellent communication, MDT collaboration, and the ability to make decisions in challenging contexts.

Qualifications

  • Senior clinical assessor role requires UK health/social care background.
  • Experience in s117 Aftercare and Mental Health Act helpful.
  • Strong assessment, care planning and MDT coordination essential.

Responsibilities

  • Conduct comprehensive assessments under the MHA s117.
  • Lead case management and complex planning.

  • Support timely discharges and care planning in care homes and patient homes.

Job description

  • Conducting annual reviews and applying principles of positive risk taking, strengths-based approach and least restrictive practice to packages of care
  • Supporting the Multi-Disciplinary Team to screen and approve new funding applications
  • Participating in discharge planning meetings, Care Programme Approach and Care and Treatment Reviews
  • Providing proactive case management to individual complex and rehabilitation clients
  • Facilitating timely discharges from acute hospitals and placements where clients are no longer gaining benefit from that service, As a Senior Clinical Assessor/Practitioner (Registered Mental Health Nurse, Registered Learning Disabilities Nurse or SWE registration with mental health specialism) in Specialist s117 Aftercare, you will be responsible for supporting clients who require care, treatment and support under the Mental Health Act, conducting comprehensive individual assessments of clients, case managing clients to ensure that their needs are effectively met and promoting Personal Health Budgets and personalisation to empower people with mental health needs to live their best lives.
  • You will need to have excellent interpersonal, presentation and communication skills to establish good rapport with people with Mental Health, Autism or both and an ability to make decisions in a challenging but supportive environment. As part of the role, you will have the opportunity to develop strong links with health and social care colleagues to ensure effective multi-disciplinary working and develop a strong understanding of local NHS and independent sector service provision in individual's own homes and in care homes. The successful candidate will be able to use specialist knowledge and clinical skills to ensure that coordination and outcomes are determined in a consistent manner to ensure effective application and interpretation of the MHA S117. Whilst experience in the Mental Health Act would be helpful, if you have the right skills and attitude, we can support you to develop the specific skills required. The Trust has a suite of educational opportunities to develop skills and we offer team training opportunities and Team Leaders who are non-case holding in order to mentor and develop our Clinical Assessors with complex cases, including safeguarding, Mental Capacity Act and Court of Protection. You will be part of a wider All Age team, comprising administration, brokerage, commissioners, Deprivation of Liberty experts, proportionate care practitioners and case practitioners who provide a wide range of expertise and support.   • Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in health or social care, Experience of assessment of health and social care needs -- care planning and coordination. Knowledge and relevant experience working with individuals who are autistic and/or have learning disabilities and/or mental health issues (LD/MH/LDAP team roles) Knowledge of complex neurological conditions and experience of working with clients with complex brain injuries.
