This post will be based in Hundens Lane, Darlington,taking referrals for the Community Nursing Team. The post holder would berequired to work with and support the team with their clinical administrativefunctions associated with the management of patients: accepting incoming tasksand referrals to the team and setting appropriate care plans. Using theautoplanner functionality within SystmOne to allocate the following daysvisits. Support the duty Sister to review allocated visits and assisit with anychanges to the visit lists.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
