Job overview

An exciting opportunity has arisen within the Nephrology and Transplant Directorate at UHW. We are looking for an enthusiastic and highly motivated nurse to join our Chronic Kidney Disease (CKD) Clinical Nurse Specialist workforce. The team care for patients holistically with CKD Stages 4-5 that may require some form of Renal Replacement Therapy in the future.

The aim of the service is to prepare, educate and equip patients with CKD, enabling them to choose their preferred form of renal replacement therapy. The CKD team ensure clear patient pathway management allowing patients to commence this treatment in a planned manner through the delivery of a seamless and comprehensive patient focussed service.

You should be caring, enthusiastic, committed and flexible with excellent interpersonal and communication skills. You will work autonomously as well as collaboratively with other specialist nurses and members of the multidisciplinary team to provide a high quality service for our patient cohort.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Main duties of the job

Working as an autonomous practitioner to devise, implement and follow up specialist treatments across the disease spectrum for patients within a specific group. To be a skilled practitioner providing expert clinical care using an extensive theoretical knowledge base and practical experience to underpin the role, while acting as a role model for the delivery of expert care.

To deliver specialist educational programmes to a range of healthcare professionals in relation to the specific disease, including medical, nursing and Allied Health Professional staff.

To lead research and audit within given speciality internally and externally to the UHB.
