Job description

The Nottinghamshire Young Persons’ CFS/ME Service promotes the management of young people by local community teams under the guidance of Consultant Paediatricians at NUH.

Early recognition of CFS/ME in Primary Care can prevent the need for admission and referral to hospital. There will however be patients for whom local services require the support of colleagues within the specialist team and in very rare cases the young person may require hospital admission. Therefore, the primary focus of this post is to deliver therapy led clinics (NICE recommendations) coordinate, develop services for young people with CFS/ME who are referred to NUH.

This Will Be Achieved Through

Education & Training.

Ongoing development and maintenance of care pathways within Primary, Secondary and Tertiary care (Health, Education and Social Care).

Sign posting young people, their carers and professionals to appropriate services.

Advice and support to professionals supporting young people with CFS/ME.

Direct Clinical Management from diagnosis to discharge (if required this will include transition to other services).

Working strategically at local, regional and national levels e.g. Commissioning of Services, Development and implementation of guidelines in collaboration with the Clinical Network Coordinating Centre (CNCC) (based in Derby).

Involvement in Research and Audit.

Coordinate and manage the care of young people with CFS/ME to meet their holistic needs, working autonomously and as part of a wider multi-disciplinary team (MDT).

Update And Develop Literature And Teaching Material For

Young People (11 – 19 years whilst in education) Younger Patients accepted in extreme circumstances and at the request of Consultant Paediatrician and NUH.

Work With Young People And Their Parents/carers, Giving Guidance In Relation To Specific Approaches And Options In The Management Of CFS/ME

To enable independent self-management of presenting symptoms.

Comprehensive assessment at first point of contact to determine management plan.

Provide appropriate rehabilitation packages in line with patient and family need.

Contribute to the development and management of the wider service provided by the Clinical Network Coordinating Centre (CNCC) for example the effective use of IT to ensure accessible information to all young people with CFS/ME and professionals.

Appropriate liaison with the young person’s named Consultant and joint consultations where appropriate.

Act as an advocate for young people with CFS/ME and their parents/carers.
