Key Duties

• Manage a well-organized patient list with a manageable workload, supported by an efficient team of clinical and clerical staff.
• Seeking a Nurse with experience in working with Chronic Disease patients.
• Flexible working options available: 3, 4, or 5 days per week, with a full-time contract covering just over 15,000 patients.
