Job Description:

Key Duties

• Manage a well-organized patient list with a manageable workload, supported by an efficient team of clinical and clerical staff.
• Seek a Nurse with experience in working with Chronic Disease patients.
• The practice offers flexibility to join for 3, 4, or 5 days per week, with a full-time contract based on hours for a patient population of just over 15,000.

Key Skills, Qualifications and Client Requirements

• Registered Nurse with active NMC pin and experience in Chronic Disease management.
• Right to work in the UK.
