Working Hours: Hybrid
Rate: £35 per hour
An experienced Social Worker is required to join East Riding’s Pathways Leaving Care Team. This is an exciting opportunity to support young people in their transition to adulthood.
Responsibilities:
As a Social Worker within the Pathway Team, you will be responsible for working with young people who are looked after and care-experienced. You will provide assessments of needs for young people preparing to leave care and develop pathway plans to meet these needs. You will offer advice and guidance to young people who are eligible, relevant, and former relevant under the Children Leaving Care Act 2000. Additionally, you will work closely with partners as corporate parents to support children in developing their life skills and working towards their aspirations.
