Job description

This nursing role within Sussex Community Foundation Trust’s Children’s Community Nursing Service (CCNS) focuses on clinical and public health care for children, young people, and families.

The Children’s Community Nurse (CCN) supports the development of the local and countywide CCNS, working with the Team Lead and senior nurses to ensure care aligns with Trust and national standards.

They deliver specialist community nursing for children and young people on the caseload, applying current knowledge and skills, and consulting clinical experts for specialist input when needed.

As a Registered Nurse - Child, the CCN promotes health and wellbeing and ensures safe, high-quality care and hold a delegated caseload, work under senior supervision, collaborating with other health professionals and partner agencies.

The role includes contributing to the teaching and supervision of nurses, healthcare assistants, associate practitioners, and students.

They are also responsible for safeguarding children in line with The Children Act (1989 & 2004) and Working Together to Safeguard Children (HM Gov 2013).

Key Responsibilities:

  • Provide competent, high-quality nursing care, support, and advice within a delegated CCNT caseload.
  • Maintain effective communication with families, carers, and partner services across health, social care, education, and hospice sectors to ensure coordinated care.
  • Contribute to the development and review of clinical protocols and deliver community-based education on equipment and treatment.
  • Accurately document all care and advice provided.
  • Act as a role model in children's community nursing and participate in relevant training and professional development.

Please note that this is a part-time and fixed term role.
