St George's University Hospitals seeks an experienced registered Children's Nurse to join the Children's Community Nursing Team (CCNT). The role centers on delivering high-quality care to children in their homes, including assessments, care planning, interventions, and meticulous documentation.

This part-time position is 33 hours per week, with weekend rosters and involvement in an end-of-life on-call rota when necessary.

Qualifications

• Registered Children's Nurse practicing in the UK.
• Maintain accurate, systematic, and timely recordkeeping.
• Work autonomously as a specialist practitioner in home settings.
• Collaborate with families and other agencies to ensure seamless care.
• Be flexible to provide a 7-day service including weekends and bank holidays.
• Participate in 24-hour on-call rota for end-of-life care when required.

Responsabilités

• Deliver high-quality nursing care to children in their home environments.
• Conduct assessments and develop care plans for individual patients.
• Implement nursing interventions and ensure thorough documentation and follow-up.
• Support patients, parents, relatives, and staff in stressful situations and provide counselling as needed.
• Collaborate with other agencies to ensure seamless delivery of home-based care.
