To provide skilled, effective and evidence based nursing care to children and young people aged 0 to 18 years with a variety of health conditions in their own home and other community settings within Northumberland and North Tyneside.
• To assess, plan, implement and evaluate packages of care for children and young people with acute and chronic illness, complex disability or palliative care needs.
• To work in partnership with the children, their families and carers and with other professionals from Health, Social Services, Education and non-statutory agencies to deliver care, teaching and training to meet the child and young persons assessed nursing needs.
• To provide care that is flexible and responsive to the individual and meets the demands of the service as a whole.
• To work with young people, with complex health needs, their familiesIcarers and others within health care to promote a seamless transition process between children and adult services.

The post holder will share with other Children's Community Nurses the responsibility for delivering skilled nursing care to children and young people aged 0- 18 years registered with a General Practitioner within Northumbria Healthcare Foundation Trust or children who are temporarily resident in those areas.

They will participate in discharge planning, multi professional/ agency meetingsIreviews and planning transition to adult services.

They will offer advice and support, teaching and training to parents, carers and others regarding nursing care for children and young people who have a health need.

In the absence of their special school nursing colleagues they will provide nursing care, advice and support to children and young people accessing education in special schools settings in Northumberland
