Job summary

A healthcare organization in Gorleston-on-Sea is seeking a Children's Community Nurse to provide acute nursing within the community. You will support children with complex medical needs, manage care plans, and collaborate closely with multidisciplinary teams. Ideal candidates will have paediatric nursing experience and the ability to work autonomously. This role offers an opportunity to make a significant impact on children's lives in their homes.

Qualifications

  • Experience of shift coordination in an acute setting within the last 3 years.
  • Paediatric nursing experience in community setting.

Responsibilities

  • Work within the Children's Community Nursing Team and hold a case load of CYP's with complex medical needs.
  • Ensure care plan from nutritional team is up to date for children that require TPN.
  • Commence and disconnect TPN whilst the child is at school as required.
  • Obtain required bloods and organise any IV therapies.
  • Maintain Health and Safety of patients and colleagues by complying with all Trust policies.

Children's Community Nurse

An exciting opportunity has arisen to work within the Children's community Nursing Team based at the JPUH. The team provide acute nursing within the community keeping children in their homes wherever possible. The CCNT support children with chronic conditions and provide their nursing needs at home preventing admission to the hospital. CCNT are often the key worker for the child and the family being the key contact. The team work very closely with the paediatricians within the JPUH and external specialist hospitals, holding weekly/ monthly catch up's.

Main duties of the job

To work within the Children's Community Nursing Team and hold a case load of CYP's with complex medical needs

To ensure the care plan from the nutritional team/ family is up to date and followed for the children that require TPN

To commence and disconnect TPN whilst the child is at school as required

To obtain required bloods and organise any IV therapies to ensure the child's condition is managed correctly.

To work closely and maintain a good working relationship with specialist team at Addenbrookes and the wider multi-disciplinary team

To be an active member of the multidisciplinary team liaising/communicating as necessary and supervising support staff

To maintain Health and Safety of patients and colleagues at all times by complying with all relevant Trust policies.
