A healthcare organization in Dursley is seeking a Children's and Young People's Health and Wellbeing Coach to support young individuals with mental health concerns. The role involves assessments, personalized support planning, and collaboration with various stakeholders. This is a full-time position with a focus on community connection and effective care pathways.
The Young Person Link Worker is a vital member of the Berkeley Vale PCN team, acting as a bridge between primary care, young people, their families, and community services. This role supports young people (typically aged between 11 and 25) who present with social, emotional, and mental health concerns by providing early intervention, assessment, personalized support planning, and onward referrals.
This position goes beyond traditional "signposting," involving mental health triage, short-term low-level support, navigation of neurodiversity pathways, and liaison with schools and external agencies. The Link Worker also plays a key role in managing referrals for local youth projects and maintaining clear communication channels between GP practices, schools, parents, and statutory services.
This is an exciting opportunity to join the Berkeley Vale Primary Care Networks team of dedicated professionals. The role is based at May Lane Surgery and covers patients at both May Lane and Cam & Uley Surgeries. Both sites are located near the market town of Dursley, Gloucestershire.
We are looking for an enthusiastic, empathetic, and well-organised individual to join our team.
Ideally, we are seeking someone who can work 30 hours per week across 4 days.
Interviews are expected to take place on 6th November 2025.
Mental Health Triage & Support
Receive referrals from GPs and receptionists via clinicaltasks or email.
Conduct triage via telephone or face-to-faceassessments with young people and/or their parents.
Signpost or refer to external services (e.g.,CAMHS, counselling, mentoring, Early Help).
Provide follow-up calls (4-12 weeks later) toensure referrals have been actioned and additional needs addressed.
Offer short-term, low-level 1:1 support whereappropriate (e.g., weekly sessions with six-week reviews).
Liaise with schools, Early Help teams, and otherprofessionals to ensure a joined-up approach to care.
Escalate safeguarding concerns in line with PCNand practice policies.
2.Neurodiversity Pathway Support
Manage referrals related to suspected ADHD/ASD,including guidance on Gloucestershire NHS pathways and Right to Choose options.
Signpost and complete referral paperwork andadministration.
Liaise with parents, carers, and schools to trackreferral progress.
Support families in navigating long wait timesand encourage collaboration with school SENCOs.
Maintain up-to-date knowledge of national andlocal neurodiversity resources and share this with practice staff.
Social Prescribing & Community Connection
Collaborate with young people to createpersonalised care plans addressing practical, social, and emotional needs.
Connect young people to local groups, mentoringschemes, counselling, youth PPG group and other non-medical support services.
Provide advocacy when needed, such as attendingappointments or assisting with service applications.
Collaboration & Process Development
Work closely with GPs, receptionists, and adminstaff to develop robust and consistent referral processes.
Share information on common themes to supportPCN planning.
Help shape future service delivery byidentifying gaps and suggesting improvements.
Record Keeping & Reporting
Maintain a secure caseload and accurate clinicalrecords.
Regularly update the practice team on patientprogress and outstanding actions.
Contribute data for PCN reporting and qualityimprovement initiatives.
