Job description

Children Community Learning Disability Nurse

To work within the Learning Disability Directorate as a member of the Children's Learning Disability Team, which currently sits within the Tameside and Glossop Learning Disability Team.

The post holder will provide specialist health and behaviour assessments to children and young people with a learning disability who are referred to the team.

Interventions will be delivered across different settings, including the person's home. There will be a requirement to liaise and work collaboratively with partners and stakeholders to ensure responsive, effective, safe, and child-centered services are delivered.

Main duties of the job
  • To work in accordance with Pennine Care NHS Foundation Trust policies and procedures at all times.
  • To provide safe, effective, responsive, efficient, and timely community nursing services to children and young people with learning disabilities, their families, and carers.
  • To undertake comprehensive assessments for children and young people with learning disabilities and complex needs, using a specialist knowledge base.
  • The post holder may be required to undertake additional training in implementing high-quality Positive Behavioural Support.
  • To offer behaviour support training to relevant teams and individuals, including teams around the child.
  • To develop intervention strategies and care plans tailored to individual needs.
  • To promote health and wellbeing, embracing the public health role, and work towards preventing ill health.
  • To support efforts in reducing health inequalities faced by people with learning disabilities.
  • To collaborate with primary and secondary care services to meet the health needs of people with learning disabilities.
  • To actively identify and manage risks associated with the care of children and young people with learning disabilities.
  • To work closely with Social Services and Child Safeguarding teams.

Job responsibilities

Person Specification
Experience
  • Registered Nurse with a specialization in Learning Disabilities
  • Post-registration training, with willingness to undertake postgraduate diploma training
  • Experience supporting children with learning disabilities
  • Experience managing risks and working collaboratively with partners
  • Knowledge of LD assessment tools and behavioral techniques
  • Ability to handle complex, sensitive, and distressing situations, including challenging behavior, abuse, and delivering bad news
  • Knowledge of child safeguarding
  • Ability to complete complex assessments and formulations
  • Experience supporting individuals with complex and challenging needs
  • Own transportation or access to mobility across the trust footprint
  • Training in positive behavioral approaches
  • Training in clinical supervision
  • Previous community nursing experience
  • Knowledge of transforming care, SEND, and EHCPs
  • Experience delivering training

This position is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975. A disclosure check will be required through the Disclosure and Barring Service to assess any criminal convictions.

Salary: £37,338 to £44,962 per annum, pro-rata.
