Job summary

A local healthcare organization is seeking a social prescriber to assess and support patients' health and wellbeing needs in York and North Yorkshire. Job description

The SPWLW works alongside a team within general practice and within thePCN and empowers people to take control of their health and well-being.

The post-holder will be an integral part of the PCN team, working acrossthe PCN, as well as part of a wider community groups and theirmultidisciplinary teams. A SPWLW supports existing groups to be accessible andsustainable and helps people to start new community groups, workingcollaboratively with all local partners.

A referral to a non-medical link worker is designed to supportpatients to be able to take a holistic approach, connecting people to communitygroups and statutory services for practical and emotional support.

Social prescribing can help to strengthen community resilience andpersonal resilience while reducing health inequalities by addressing the widerdeterminants of health such as debt, poor housing and physical inactivity byincreasing peoples active involvement with their local communities.

This role can be particularly beneficial to patients with long-termconditions, those with mental health issues and those who are lonely orisolated or who have complex social needs which affect their wellbeing.

Main duties of the job

Providecare within own competence and seek advice or refer to colleagues whenappropriate, ensuring that practice is effective, relevant, evidence-based andof a high standard.

Developknowledge of local services to enable patients to access a range of services tomeet their needs and ensure they are engaged and connected with their localcommunity and other organisations and groups to make best use of resources.

Takereferrals from the local network(s) and work with the referrer and the patientto plan a holistic based approach to care to include a personalised care plan.

About us

Theaward-winning Selby Town PCN consistsof four practices with a combined population of approximately 50,000 patients,covering the large geographical area surrounding Selby and Escrick. Thesuccessful candidate(s) will be employed by Selby Prime Care Ltd who are contracted to provide PCN activities to thepractices in the locality.

We are looking to recruit 3 x whole time equivalent posts to support patients& staff across the four practices in the PCN: Beech Tree Surgery,Escrick Surgery, Posterngate Surgery and Scott Road Medical Centre. The PCN hasa Clinical Director, two Clinical Lead GPs and is managed by the PCNOperations Manager & HR Lead. We work in increasingly collaborativepartnerships across a number of workstreams, which include important priorityareas of mental health, frailty, urgent care and health inequalities &population health. Our team consists of a diverse multi-professional workforcewhich includes pharmacists, MSK practitioners, mental health practitioners,care coordinators, a frailty therapy team, an urgent care coordinator, abusiness intelligence analyst/digital lead, a transformational lead and anurgent care team of health care professionals.

Job responsibilities

Assesspatients health and wellbeing needs to ensure they can be met by services andother opportunities available within the community.

Providepersonalised support to individuals, their families and carers to take controlof their health and wellbeing based on the persons priorities and widerdeterminants of health.

Collaboratewith patients to produce a simple-to-follow personalised care plan to addresspatients health and wellbeing needs by introducing or re-connecting patientswith access to community groups or statutory services. This can be based ontheir priorities, interests, values and motivations, including what they canexpect from participation with groups, activities and services they willconnect with through social prescribing referrals.

Evaluatehow the care and support plan(s) meet the individuals health and wellbeingneeds, and take a holistic approach based on the persons priorities and thewider determinants of health.

Helppatients identify wider issues that impact on their health and wellbeing, suchas debt, poor housing, unemployment, loneliness and caring responsibilities.

Promotethe role of social prescribers in-house and the wider community.

Need tomanage and prioritise their own caseload in accordance with the health andwellbeing needs of their local population needs, referring patients to otherhealth professionals within the PCN as necessary.

Developeffective working relationships with local voluntary, community and socialenterprise organisations (VCSE) and community groups make and receive timely,appropriate and supported referrals.
