Job summary

Service Care is seeking a dedicated Community Care Worker for the Children’s Services team. You will support social workers, assess needs, coordinate services and contribute to care planning to improve outcomes for vulnerable children and families.

You will work under guidance, manage a caseload of less complex cases, ensure care plans are implemented, and liaise with partners across education, health and voluntary sectors to deliver person-centred support.

Qualifications

• Direct experience working with children in social care, education or health care.
• Understanding of child development and needs of children and families.
• Ability to identify and assess needs of children and families.
• Good written and verbal communication skills.
• Strong organisational skills and workload management.
• Ability to work with professionals, agencies and community organisations.
• Good IT skills and accurate record-keeping.
• Confidentiality, data protection and equal opportunities understanding.

Responsibilities

• Identify and assess needs of children and families to provide appropriate support.
• Undertake holistic assessments for less complex cases and assist with care plans.
• Arrange and supervise family contact where required.
• Manage a caseload under supervision and review cases regularly.
• Review service user progress and changing needs.
• Liaise with social workers, education and health professionals and agencies.
• Coordinate internal and external services and resources.
• Provide practical support and transport where needed.
• Maintain accurate case records and prepare reports per policies.
