Job summary

Oldham Council is seeking a Child and Family Practitioner to contribute to the Integrated Children and Family Service. Based in one of Oldham’s Family Hubs, you will support universal Start for Life delivery and targeted interventions to families, children and carers.

Key duties include observing child development, promoting health messages, and making referrals to Family Hub services. The role requires Level 3 in childcare and experience in health, education or social care settings.

Qualifications

  • Relevant Level 3 qualification in childcare, early years, health & social care.
  • Experience of working directly with children, young people and adults in health, education or social care settings.
  • Ability to observe, identify and assess the needs of children, young people & families.
  • Knowledge of early child development and the Early Years Foundation Stage.

Responsibilities

  • To support the delivery of the Healthy Child Programme 0-19 years under supervision of the Health Visitor/Senior Child & Family Practitioner.
  • To observe, assess and enhance the wellbeing, development and behaviour of children to support readiness for school.
  • Promote public health messages, healthy lifestyles and emotional health and wellbeing for children, young people, families.
  • Make referrals for families to access services and resources in the Family Hub network.

Role Overview:

Working as part of Oldham’s Integrated Child & Family Service the post holder will contribute to the delivery of the universal Start for Life and wider Family Hub offer including delivery of universal and targeted interventions in Family Hub buildings and through our outreach and digital offer.

Key Responsibilities:

  • To support the delivery of the Healthy Child Programme 0-19 years under the supervision of the named Public Health Nurse (Health Visitor) / Senior Child & Family Practitioner. This will include undertaking observation-based assessments on child development at 9-12 months & 2.5yrs utilising screening tools i.e. ASQ-3, Wellcomm etc
  • To observe, assess and enhance the wellbeing, development and behaviour of children to support children’s readiness for more formal learning at school.
  • Promote public health messages, healthy lifestyles and emotional health and wellbeing for children, young people, families promoting self-care and access to health care services, including the Healthy Child Programme.
  • Make referrals for families to access services and resources in the Family Hub and network of Family Hub services as appropriate to promote healthier lifestyles and build capacity in individuals

Key Requirements:

To succeed in this role, you will need as a minimum:

  • Relevant Level 3 qualification in childcare, early years, health & social care
  • Experience of working directly with children, young people and adults in health, education or social care settings
  • Ability to observe, identify and assess the needs of children, young people & families
  • Knowledge of the early child development and the Early Years Foundation Stage to support the planning of appropriate activities and interventions to support children’s individual developmental needs
  • Willing to work flexibly within the organisation. Agile working and supporting cover requirements
