Job summary

Mersey Care NHS Foundation Trust is seeking a motivated mental health practitioner to join the Warrington CYPMHS Team. The successful candidate should work well autonomously and in a team, delivering care and therapy to children and young people and their families using the Think Family approach.

We encourage applications from those with experience and offer a robust preceptorship programme to develop your skills within the first 12 months.

Qualifications

• Experience of working with a range of people with mental health problems.
• Effective care coordination.
• Care planning and evaluation.
• Quality and clinical audit.

Responsibilities

• Coordinate the care of service users within the care programme approach and under supervision.
• Develop and maintain care plans that reflect assessed needs and involve relevant staff.
• Liaise with family, carers and significant others during assessment and care planning.
• Develop risk management plans and ensure all key people are aware of and act on them.
• Direct unqualified staff to ensure delivery of high-quality care.
• Participate in setting standards based on evidence and audit findings.
• Communicate with the multi-disciplinary team to share information impacting care.
• Attend training and safeguarding processes and follow Mersey Care policies.
