Job summary

Wiltshire Council's Continuing Healthcare Team is seeking a Level 3 Social Worker to assess adults for NHS CHC funding and provide specialist guidance. You will work with health and social care partners to determine eligibility and secure appropriate support.

The successful candidate will be a registered Social Worker with Social Work England, experienced with safeguarding and complex care needs, and committed to ongoing professional development and high-quality client outcomes.

Qualifications

• Qualified and registered Social Worker with Social Work England.
• Up-to-date knowledge of safeguarding, health and social care legislation and policy.
• Experience in supporting vulnerable adults and commitment to ongoing professional development.
• Strong communication skills.

Responsibilities

• Assess individuals with complex health and care needs and support CHC eligibility decisions.
• Ensure appropriate funding arrangements are in place and provide CHC advice.
• Conduct person-centred assessments and manage risks and safeguarding concerns.
• Help people access required support and promote independence and positive outcomes.
