Zusammenfassung

Hampshire County Council is seeking a registered Social Worker for its Continuing Healthcare (CHC) team. You will work within a multidisciplinary environment to manage a diverse caseload across adults with mental health, physical and learning disabilities, and older adults.

You will assess and plan care following Discharge to Assess, applying the Care Act 2014, and collaborating with families to secure appropriate care arrangements.

Qualifikationen

• Social Work qualification and SWE registration.
• Experience in UK Adult Social Care.
• Knowledge of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care (July 2022).
• Understanding of social work legislation and statutory duties.
• Commitment to values and compassionate practice.

Aufgaben

• Support individuals with onward care journeys after hospital Discharge to Assess.
• Undertake assessments in line with the Care Act 2014.
• Engage with multidisciplinary teams to support NHS CHC assessments.
• Collaborate with individuals and families to arrange suitable post-assessment care.
• Undertake mental capacity assessments.
• Manage challenging and complex cases.
