Job summary

Hampshire County Council seeks an experienced Social Worker to join the Continuing Healthcare (CHC) team. You will work within a multidisciplinary environment, handling a diverse caseload across adults with mental health, physical and learning disabilities, and coordinating care following hospital discharge.

The post requires SWE registration and UK-based Adult Social Care experience; familiarity with NHS CHC/NHS FC 2022; a strengths-based approach and knowledge of statutory duties.

Qualifications

• SWE registration is required for the role.
• UK-based experience within Adult Social Care is essential.
• Knowledge of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care (July 2022 revised).
• Solid understanding of applying a strengths-based approach in practice.
• Awareness of social work issues, legal frameworks and statutory duties.

Responsibilities

• Support individuals with their onward care journey following a Discharge to Assess period from hospital.
• Undertake assessments in line with the Care Act 2014.
• Engage with multidisciplinary teams to support NHS CHC assessments.
• Collaborate with individuals and families to transition to suitable care provision after assessment.
• Undertake mental capacity assessments.
• Manage challenging and complex cases.
