We have a vacancy within our service for a motivated and enthusiastic Learning Disability Nurse to join our team.

The role involves assessing eligibility under the NHS National Framework for Continuing Healthcare and Funded Nursing Care 2009 (revised 2022), planning and commissioning care packages (including Personal Health Budgets), supporting individuals, families, and professionals through the process (including transition from child to adult services), and reviewing eligibility and care packages.

Main duties of the job

We seek an experienced nurse practitioner working with people with learning disabilities—an essential characteristic.

NHS Continuing Healthcare (CHC) is a package of care arranged and funded by the NHS for individuals with complex ongoing healthcare needs outside hospital.

Devon Partnership NHS Trust is responsible for decision-making and commissioning, working closely with Local Authorities on behalf of Devon ICB.

Successful candidates will have excellent communication and assessment skills, be resilient, able to work autonomously and as part of a team. This is a community-based role, based in Exeter and East Devon, but requires travel across Devon and occasionally out of the county.

The team values a fast-paced, holistic approach to nursing, focusing on the needs of the person. The role is autonomous and challenging but offers supportive colleagues.

Engage with stakeholders including staff, service users, families, carers, and agencies to ensure understanding of care processes related to Continuing Healthcare, Funded Nursing Care, and Section 117 aftercare.

Communicate complex and sensitive information to:

  • Service users and families regarding assessment, care planning, treatment, and review
  • Multidisciplinary teams to ensure coordinated care
  • Primary care teams about client care
  • External agencies involved in care provision (voluntary, private, police, social services, housing, employment)
  • Safeguarding, DOLS teams, CCGs, and Devon County Council regarding concerns with care packages
  • Other professionals and agencies as needed
  • Care Brokerage as appropriate

Analytical and Judgement Skills

  • Use clinical judgement to support procurement and commissioning of care packages, ensuring community-based support, hospital avoidance, and timely discharge.
  • Assess complex situations to determine appropriate care, supporting verification and funding decisions.

Planning and Organisational Skills

  • Organise multidisciplinary meetings, including DST and Section 117 reviews
  • Lead contentious decision-making processes regarding CHC or FNC eligibility involving MDT, service users, families, and legal representatives
  • Plan and organise care packages and reviews within required timeframes
  • Ensure care/support meets assessed needs and outcomes
  • Manage own workload and support team members, prioritising effectively
