An exciting opportunity has arisen to join Coventry and Warwickshire Integrated Care Board to come and work in our Continuing Healthcare Team within Coventry. The service assesses patients for eligibility and secures placements for those funded by CHC, S117, NON CHC and D2A across Coventry.
You will be joining a team that has transformed the operational service to one which now meets NHSE Quality Targets, discharges patients within 48hrs from an acute trust and has improved patient experience and reduced complaints by 80%. Coventry and Warwickshire Integrated Care Board is looking for a Clinical Assessor - RGN to join their Clinical Assessment Team, with responsibility for case management: being the single point of contact for the patient and their family supporting them through the assessment process. You will also work with the internal / external teams to secure an appropriate placement / package of care. In addition you will undertake regular care and eligibility reviews with local authority partners.

Mobility
Employees may be required to work at any of the other sites within the organisation subject to consultation. The organisation is in a period of rapid change due to developments and rationalisation of services. This will lead to modification of structures and job descriptions. The post holder will be expected to co-operate with changes subject to consultation, at any time throughout the duration of your contract
