Main duties of the job

As a Band 6 Learning Disabilities Nurse in our AACC team, you will play a crucial role in coordinating and delivering comprehensive healthcare services to individuals with learning disabilities. Your expertise will ensure that patients receive the best possible care, tailored to their unique needs.

This role offers a variety of responsibilities with no two days being the same. You will use your knowledge and clinical skills to be involved in many aspects of the teams functions, including:

• Conducting comprehensive assessments of patient care needs
• Co-producing individualised care and support plans to meet the complex needs of patients
• Coordinating the process of sourcing appropriate care provision.
• Liaising with key stakeholders to ensure timely and safe patient transfers.
• Acting as a contact point for family and patient representatives.

Job responsibilities

• Thepractitioner will utilise the national assessment tools and adhere to therequired timescales, co-ordinate, participate and support the assessmentprocess with the multidisciplinary team.
• Toco-ordinate and undertake assessments. To collate information and assessments,gathered from other Health and Social Care professionals, patients and carersand make judgements using this information. Taking into account opinions frommedical staff, consultants and specialist practitioners. This may involvecomplex and sensitive health and social care issues, patients diagnosis andprognosis. To maintain appropriate written records and communicate informationto all relevant parties whilst maintaining appropriate confidentiality of thepatient record.
• Torestore and maintain independence, promote individual choice and selfdetermination, enabling clients to live in the community and supporting theirpreferences, whenever feasible. The use of Personal Health Budgets will assistin supporting this objective and will be delivered by the practitioner.
• Maintaining a caseload of patients eligible for FundedNursing Care and Fully Funded Continuing Healthcare, including the assessmentand reviewing of all care provision.
• Toeducate, inform and involve clients carers and professionals on the NationalFramework for Continuing Health Care and NHS Funded Nursing Care (2018) andmatters of local service provision.
• Tobe actively involved in complaint investigations assisting the Lead Nurse andHead of Service as required.
• Contributein the response to appeals supporting the Lead Nurse in completing needsportraits and attending both local and regional appeal panels.
• Ensurethe Head of Department, Lead Nurse and the Commissioning team are kept fullyaware of any cause for concern in any care environment; liaising as requiredwith the ICB Quality Team.
• Beaware of the safeguarding vulnerable adults policy and procedure and attendmultidisciplinary meeting as required.
• Tomaintain registration, skills and knowledge including an awareness of policies,procedures, legislation laid out by the relevant professional bodies.
• Workwithin the clinical guidelines and policy guidelines relevant to practice.
• Participatein research, audit, clinical and peer supervision on a regular basis to developappropriate personal objectives and a lifelong learning plan
• Todevelop and maintain multi professional/agency networks representing the ICB.
• Responsiblefor the teaching and dissemination of information to students, social carestaff, health professionals, and independent sector about NHS Continuing Careand Funded Nursing Care Contributions, on an individual and group basis.
• Toprioritise and manage caseloads, under the direction of the Lead Nurse.
• Tocontribute to the effective running of the department, deputising forcolleagues and Lead Nurse.
• Tocall, attend or lead team meetings, continuing care meetings andmulti-disciplinary meetings, including case conferences and discharge meetingsto ensure continuity of care across agencies; thus, improving patient andfamily experience.
• To promote effective workingarrangements with care homeowners, social services, community nursing and acutesector staff.
• Toundertake and comply with all mandatory training.
• Toundertake any other duties commensurate with the grade and responsibilities ofthe post.
