Job description

Care Practitioner - NHS CHC

Location: Fully Remote (UK)

Contract: Permanent, Full-time (37.5 hrs)

We're looking for an experienced Registered Nurse CHC assessor to join a leading organisation delivering Continuing Healthcare (CHC) assessments. This is a fantastic opportunity for someone seeking a secure, long-term career in a supportive environment that values work-life balance, career progression, and high-quality patient-centred care.

About the Role

Working within the National Framework for NHS Continuing Healthcare (CHC) and the Care Act 2014, you will:

  • Chair Multi-Disciplinary Team (MDT) meetings held online (MS Teams or Zoom) with patients, families, and care providers.
  • Complete CHC assessments and reviews using the Decision Support Tool (DST), providing clear eligibility recommendations.
  • Create person-centred care and support plans.
  • Liaise with local authority representatives and other professionals to gather evidence and ensure robust decision-making.
  • Work to agreed Key Performance Indicators (KPIs) for quality, productivity, and service delivery.

This is a fully remote role, giving you the flexibility to balance professional and personal commitments while still making a tangible difference to people's lives.

What We're Looking For

We welcome applications from Registered Nurses (NMC-registered) with at least 12 months' post-registration experience and:

  • Previous experience completing CHC assessments and reviews within the NHS or similar settings.
  • Strong understanding of the CHC Framework 2022, Care Act 2014, and Mental Capacity Act 2005.
  • Excellent written skills to produce clear, person-centred, evidence-based reports.
  • Confident communication and empathy when dealing with sensitive, often complex health and social care matters.
  • Ability to plan and organise workload, meet KPIs, and work collaboratively with other professionals.
Why This Role Stands Out

We know that choosing a permanent role is about more than salary. This position offers the stability of a secure employer and the opportunity to develop your career in a business that values and supports its people.

You'll benefit from:

  • Competitive salary aligned with your experience
  • 25 days' annual leave plus 8 bank holidays (holiday entitlement increases with service)
  • Contributory pension scheme
  • Private healthcare and dental insurance
  • Life assurance (4× your salary) and income protection for peace of mind
  • Option to buy extra annual leave
  • Blue Light Card, gym discounts, and salary-sacrifice schemes
  • Wellbeing programmes - including fun activities, company days, and access to an Employee Assistance Programme
  • Volunteer Days to give back to the community
  • A supportive workplace that values care, honesty, inspiration, and guidance

This role is perfect for someone who wants career progression in a stable, growing organisation, while enjoying a healthy work-life balance in a fully remote environment.

If you're an experienced CHC nurse looking for your next step, one that offers stability, professional development, and a better balance of work and life, we'd love to hear from you.
