Overview

Align recruitment are looking for multiple Band 6 CHC workers to work for a trust in South west England.

Responsibilities

• Remote working with the ability to complete remote assessments; face-to-face assessments may be required very limited.
• Conduct CHC assessments and Reviews.

Qualifications

• DoLS worker: experience completing community DoLS and competent at MCA.
• PHB worker: knowledge of completing Care and Support Plans and PHB guidance.
