Job description

We are seeking an experienced and highly motivated Children & Young Peoples Continuing Care Nurse to join our Wolverhampton team. You will play a pivotal role in assessing, planning, and co-ordinating care for children and young people with complex health needs, ensuring delivery of high-quality, personalised, and family-centred services

About You

You will be a Registered Childrens Nurse (RSCN/RN Child) with substantial post-registration experience, including working with children & young people with complex and long-term health needs. You will have a thorough understanding of the National Framework for Children and Young Peoples Continuing Care and be confident working across organisational boundaries to achieve the best outcomes.

Main duties of the job

You will work within the NHS Continuing Care framework to:

  • Conduct holistic assessments with highly complex health needs to make evidence-based eligibility recommendations.
  • Develop and review care packages in partnership with families, education providers, social care, and other health professionals.
  • Source and commission tailored support packages
  • Provide compassionate, sensitive communication to children, young people, and their families, including delivering complex information and managing expectations regarding care provision.
  • Offer expert advice and guidance to the All-Age Continuing Care team, ICB and wider health and social care community.
  • Ensure that care meets the highest standards of safety, quality, and compassion.

Job responsibilities
  • Thepost holder will be required to provide a high-quality resource of professionaladvice, guidance and expertise to members of the Individual Care Team, ICB andwider health and social care economy
  • Autonomouslymanage a case load of individuals with highly complex health needs providingsignificant clinical guidance to providers.
  • Ensurethat all assessments are holistic in nature and utilise The National Frameworkfor Children and Young People Continuing Care.
  • Tohave an expert level of knowledge regards The National Framework and its applicationin practice. To be able to impart this knowledge to a variety of audiences.
  • Toprovide complex and sensitive information to patients, relatives/representatives and often work in potentially hostile environments, relayingdifficult decisions regarding care provision and managing patient and familyexpectations.
  • Providecontinuity of care throughout the period of care and support. This willencompass close working with social workers, other health professionals andcare providers.
  • Attendall meetings related the patient which may impact on the care provision andcosts to meet identified needs
  • Liaisewith multiple professionals involved in the patients care to ensure that acomprehensive understanding of all identified needs is available. Ensuring thateach patient has a care plan that addresses all their identified needs and canbe used to assist assessors when completing assessments/reviews. Source themost appropriate placement/provider.
  • Coordinateand improve health SEND planning and practice. This will involve effectiveliaison with local authorities in the undertaking of Education, Health and CareAssessments within the SEND processes.
  • Toprovide relevant information to the SEND Programme Officer to facilitateappropriate decision making regarding a CYPs eligibility for continuing care.This will include the completion of the decision support tool and the collationof multi-disciplinary assessments.
  • Tocontribute to the strategic direction for the local health services in meetingstatutory requirements for SEND, identifying priorities for development andfacilitating improvements.
  • Facilitatethe development of personal health budgets as a vehicle for the delivery ofcare services to CYP with complex care needs.
  • Supporteffective and efficient transition of CYP to appropriate adult care services
  • Presentcomprehensive written reports to ICB Eligibility Panel following any reviewscompleted once patients are in receipt of agreed care provision that identify achange in the level of care required. This includes being able to demonstrateimprovements that may result in a reduction of care provided or a change thepatients eligibility for Continuing Care.
  • Tosupport the resolution of any disputes with CYP, carers and other commissioners
  • Tobe responsible for negotiating care package costs and the monitoringof packages of fully funded care which are in excess of £1000 perweek. To be responsible for ensuring these care packages are reviewed to anagreed time scale, identified by the Head of CHC and that these are recorded onthe ICB database.
  • Identifyand process any safeguarding and quality of care issues and takeappropriateaction to ensure that peoples welfare is protected and that thequalityand standard of services provided are at a level that is in line with ICBrequired standards. Close liaison with the ICB Quality Team is necessary toensure ICB funded individuals are in receipt of high quality, safe care.
  • Developand maintain effective working relationships with clinicians, generalpractitioners and providers in order to ensure continuity of care and addresspatient needs.
  • Provideinformation and support to the senior managers when considering the mostappropriate provider to address patients identified needs and to actautonomously when required to agree care packages in excess of the usual ICBcost.
  • Tomaintain a formal record of all cases and ensure that all relevant clinicalinformation is available and updated regularly, both electronically and inpatient files.
  • Supportthe Head of Service in working with partner agencies and other stakeholders indeveloping the local market for services to ensure that it reflects therequirements of individuals and is able to meet current and future needs
  • Tocall and chair multi-disciplinary meetings, including case conferences anddischarge meetings to ensure continuity of care across agencies; thus,improving patient and family experience.
  • Providedata in order that the ICB can continuously monitor care delivery and identifyhow performance of providers can be improved. Working closely with colleaguesin the ICB Quality Team to assure the quality of care received by NHS fundedindividuals.
  • Tohave budgetary monitoring responsibilities and to ensure that the Head ofService is informed of any identified budget pressures or financial risks thatmay impact on the overall service budget. The post holder will need todemonstrate both quality of care and value for money in all placements.
  • Todevelop policies and procedures, as requested, which will support the overalldelivery of Children and Young People within the ICB.
  • Tocontribute to and deliver training on Children and Young Peoples complexhealthcare to a variety of audiences particularly in health and social care
  • Toprovide clinical support and mentoring to the nurse assessors within the teamand provide additional support and training as required by the Lead Nurse.
  • Towork within the NMC Code and to meet requirements for revalidation.
  • Toundertake and comply with all mandatory training.

Qualifications
  • NMC Professional Registration
  • Evidence of continuous professional development
  • Academic/professional qualification at masters level or equivalent level of experience
Skills and Knowledge
  • High level of working knowledge of the relevant legislation and theories underpinning the provision of services to the relevant client group
  • Demonstrable experience and knowledge of current key policies and priorities within the NHS, especially within Clinical Commissioning Groups
  • A clear knowledge and understanding of service provision available locally, regionally and nationally for the client group
  • Understanding of relevant current NHS and social care market mechanisms
  • Good working knowledge of childrens safeguarding
  • Awareness of Data Protection and confidentiality issues
  • Working knowledge of Microsoft Office with standard keyboard skills
  • Effective interpersonal and negotiation skills enabling effective communication with clients, other professionals, colleagues, partner agencies, providers and carers.
  • Excellent written and verbal communication including providing and receiving complex, sensitive or contentious information
  • Ability to prioritise and to work effectively on own initiative
  • Ability to work in hostile environments and deal with conflict on a regular basis
  • Understanding of the commissioning/procurement process for Childrens continuing care packages
Experience
  • Significant experience gained over a number of years in a complex, multi-disciplinary environment
  • Extensive experience of case management with service users who have complex health needs
  • Clear knowledge and understanding of The National Framework for Children and Young Peoples Continuing Care and its application in practice
  • An understanding of the Mental Capacity Act 2005 and its application
  • An understanding of commissioning for complex health needs in the context of Continuing Care for Children and Young People
  • Experience of monitoring a budget and understanding of financial systems and processes
