A regional healthcare organization is seeking a Band 6 Case Manager and Assessor to join their Continuing Healthcare team in York. In this role, you will manage and assess cases for individuals eligible for NHS funding, working collaboratively with a multidisciplinary team. Candidates should be qualified RGN/RMN/RNLD and ready to support positive changes in vulnerable adults' lives. This position offers flexible working arrangements.
An opportunity within the ICB for colleagues who are qualified RGN/RMN/RNLD who are currently fulfilling a non-clinical or clinical role to consider a change in direction and join the new Continuing Healthcare team. We are looking to recruit a dynamic and innovative Band 6 Case Manager and Assessor to work as part of the team, delivering effective case management, assessment and reviews across the geography for persons who may be eligible for NHS funding. The successful candidate will join a newly formed integrated team of clinicians, social workers and administrative staff.
There are 2.0 WTE posts available in the North Yorkshire and York team. All flexible working requests will be considered.
This role is based in the North Yorkshire and York area, with two 2.0 WTE posts available in the team.
