We are looking to recruit a dynamic andinnovate Band 6 Case Manager and Assessor to work as part of the team,delivering effective case management, assessment and reviews across thegeography for persons who may be eligible for NHS funding. The successfulcandidate will be joining a newly formed integrated team of clinicians, socialworkers and administrative staff.
There are 2.0 WTE posts available in the Hull& East Riding Team.

Part time and full time posts will beconsidered.
The team is able to offer an upskilling periodfor those who apply but may not have all the required skills and experience tobegin the role immediately. This will likely take 4 months to upskill andsupport the individuals who are successful to post. The upskilling will includeundertaking digital training through NHS England, alongside shadowing andsupport from existing Complex Care, including CHC, colleagues.
This role will work with the seniorclinical team and Operational Manager to deliver the 'end to end' ContinuingHealthcare pathway. This includes eligibility assessment, review, casemanagement and duty. As a member of the team, you will be working with a groupof highly skilled people knowledgeable and passionate about the ICB achievingits statutory duties as set out in the national Continuing Healthcareframework.
As a Case Manager and Assessor, you will alsocontribute to the ICB's essential operational objectives and duties, whichincludes the achieving of key national performance indicators the delivery ofgood quality service and care, engage in meaningful supervision, and help tobuild and shape the development of new guidance for the delivery of CHC acrossthe combined footprint of the Humber and North Yorkshire ICB. You will workwith families, our local authority partners and the wider MDT

NHS Humber and North Yorkshire Integrated Care Board (ICB) is responsible for planning and arranging the provision of NHS services to meet the diverse health needs of a population of 1.7 million people. Our area reaches over 1,500 square miles and includes the cities of Hull and York and the large rural areas across East Yorkshire, North Yorkshire and Northern Lincolnshire.The ICB is part of the Humber and North Yorkshire Health and Care Partnership, one of 42 Integrated Care Systems (ICS) across England to meet health and care needs, coordinate services and plan in a way that improves population health and reduces inequality between groups. The Health and Care Partnership comprises of NHS organisations, local councils, health and care providers and voluntary, community and social enterprise (VCSE) organisations.
Thepostholder will provide comprehensive clinical advice, assessment and casemanagement to NHS Continuing Health Care and NHS Funded Nursing Care functionswith particular responsibilities for people who have complex health care needs,this includes all categories of needs e.g. physical, end of life, Mental Healthand Learning Disability needs.
Thepostholder will follow the process set out by the NHS National Framework forCHC & FNC to provide comprehensive assessment, case management, review andfollow up on all CHC & FNC functions for the CHC Team. In addition, theywill provide support and advice to other workers, MDTs, carers and patients asrequired in implementing the National Framework for CHC &FNC, contributingto the ongoing training of other colleagues as necessary.
Thepost holder will procure safe and appropriate care packages based on individualneeds and support the Local Authority with jointly funded packages of care.
Thepostholder will be required to undertake complex assessments to identifyeligibility for Continuing Health Care and Funded Nursing Care by completingthe National Decision Support Tool. Gather evidence to support complex clinicaljudgements, which takes into account the knowledge and advice of otherprofessionals and individuals / relatives views.
Inaddition, the postholder will assist with complaints, IRPs and retrospectivecases from a clinical viewpoint.
Thepostholder will ensure risks are observed and actioned and are aware of thelocal governance arrangements and work within this when carrying out duties onbehalf of the CHC team.
Thepostholder will be expected to keep accurate, high-quality documentation, ensuringinformation flow to the business and admin team is up to date in order thatthey can manage data and process payments.
Thepostholder will provide and receive highly complex clinical and socialinformation relating to patients and explain this to MDT meetings, teammeetings, other agencies patients and carers where there may be barriers tounderstanding and resistance to messages being accepted-i.e. if care cannot berecommended or funded.
Thepostholder will manage difficult relationships from other parties which ariseout of case management, reporting difficulties and seeking support to manage asrequired. They will work flexibly within a team and communicate well with othercolleagues, ensuring all records are comprehensive and that colleagues arebriefed on potential issues, which may arise during any planned absence.
Ensureany safeguarding or examples of poor-quality care are appropriately respondedto reported and followed through, working closely with the safeguarding team /quality and contracting teams.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
