Job summary

The Older peoples Inpatient Service provides inpatient care, assessment and treatment to service users living with dementia and service user with functional mental health needs such as diagnosis of depression or psychosis. We aim to provide effective person cantered care to all service users with our service and offer an individualized care plan that both aid in their recovery and promote wellbeing. We also aim to work in partnership with service users to both maintain a person's skills and personhood but also work in promoting coping strategies.
The post holder will act as a lead nurse for an identified caseload of patients ensuring that all appropriate documentation is completed in line with Trust Policies and local procedures.
The post holder will provide assessment of patients admitted to the unit in line with the agreed MDT assessment format, this includes completion of risk assessment tools, observation and engagement requirements and identifying unmet need.
The core purpose of this role is the Planning, implementation and evaluation of a person cantered plan of care with a range of staff working within the MDT for the unit, for their designated patients
The post holder will manage the care of patients detained under the Mental Health Act 1983, ensuring all relevant documentation and processes are applied, as per legal requirements.

Main duties of the job
  • Actively participate in the Trust and Unit structures and systems to enable professional development and growth e.g Clinical Supervision, Management Supervision and Appraisals.
  • Actively participate in team discussions to contribute to service improvements and developments.
  • To be committed and positive towards innovation and change.
  • To play an active role in supporting implemented change.
  • To maintain standards set by the senior clinicians.
  • Lead in developing and maintaining the delivery of nursing care to promote health and wellbeing.
  • To participate in the induction and support of new staff and students from a variety of different professional backgrounds under the supervision of senior colleagues.
  • Contribute data for local performance indicators.
  • Follow the inpatient Care Pathway as specified for the functional mental health wards and to report on variances and feedback as necessary.
  • The post holder on occasions may lead or be involved in project work or contribute ideas/participate in subgroups to provide feedback on specific topics that they currently have experience within their scope of professional practice
