Provides personal care and related services per an established care plan, focusing on patient comfort and needs at home.

Essential Functions

• Assist with activities of daily living: hygiene, ambulation, eating, dressing, and meal prep
• Support patient-specific activities as instructed by a nurse, including dressing reinforcement, exercises, and diet management
• Perform homemaking tasks: cleaning, laundry, and organizing

Qualifications

• High school diploma or G.E.D. preferred
• Minimum six months experience as a Personal Care Assistant
• Competency Evaluation compliant with state law
• CPR certification recommended
• Proficiency in English
• Valid driver’s license and reliable transportation
• 30-40 hours of basic care training required
