Mae’r Cynllun Cenedlaethol i Atgyfeirio Cleifion i Wneud Ymarfer Corff (NERS) yn rhaglen atal a rheoli cyflyrau cronig sy’n anelu i wella iechyd a lles oedolion anweithgar. Mae’n darparu rhaglen 16 wythnos o weithgarwch corfforol i unigolion sy’n cael eu hatgyfeirio gan weithwyr iechyd proffesiynol y GIG, gan ddefnyddio technegau newid ymddygiad i wreiddio arferion gweithgarwch cadarnhaol.
Ar ôl eu hatgyfeirio, mae cleifion sy’n bodloni’r meini prawf yn cael eu gwahodd i’w canolfan hamdden leol ar gyfer asesiad cychwynnol gyda gweithiwr atgyfeirio ymarfer corff proffesiynol cymwys. Fe fyddant yn cael rhaglen ymarfer corff wedi ei thilwra, dan oruchwyliaeth am 16 wythnos, ac fe fydd eu cynnydd yn cael ei adolygu ar gyfnodau allweddol.
Fel Gweithiwr Ymarfer Corff Proffesiynol byddwch yn chwarae rhan allweddol wrth gyflwyno’r rhaglen Atgyfeirio Cleifion i Wneud Ymarfer Corff yng Nghonwy. Bydd deiliad y swydd yn cynnal ymgynghoriadau cychwynnol ac adolygiadau parhaus gyda’r holl gleientiaid; goruchwylio sesiynau campfa; a hwyluso dosbarthiadau ymarfer corff grŵp fel rhan o’r swydd.

An opportunity has arisen for a suitably qualified and dedicated Casual Exercise Professional to join our team within Conwy’s Leisure Services Department.
The National Exercise Referral Scheme (NERS) is a chronic condition prevention and management programme which aims to improve the health and wellbeing of sedentary and inactive adults who are at risk of developing or who have an existing chronic condition. It provides a 16-week programme of physical activity to individuals referred by NHS health professionals, using behaviour change techniques to embed positive physical activity habits.
Once referred, patients that meet the criteria are invited to their local leisure centre for an initial assessment with a qualified exercise referral professional. They will be offered a tailored, supervised exercise programme for 16 weeks and their progress will be reviewed at key points.
As an Exercise Professional you will play an integral role in the delivery of the Exercise Referral programme in Conwy. The post holder will conduct initial consultations and ongoing reviews with all clients; supervise gym sessions; and facilitate group exercise classes as part of the role.
A recognised Exercise Referral qualification is essential for this post.
