About the Role

The Caseworker is responsible for the provision of case management for our Clients across a range of therapeutic services including residential and outreach programs. Working in collaboration with the direct care team the Caseworker will develop case plans in accordance with the required standards, strength based principles and good practices.

Primary accountabilities of the role of Caseworker include, but are not limited to:

  • Develop and maintain professional networks within the agency and community services to improve outcomes for our clients.
  • Provide specialist advice to residential and support staff on the engagement of our clients in the case management and treatment process.
  • Provide specialist case management advice to case management staff in planning, implementing and monitoring the effectiveness of the case plans for our clients.
  • Assist in the provision of critical incident debriefing and ongoing support for staff and our clients where appropriate.
  • Actively participate in and contribute to the Care Team working with our clients.
  • Undertaking holistic assessment of our clients including their safety, permanency and wellbeing needs.
  • Coordinating and monitoring support and services for our clients in residential services and outreach programs within a therapeutic trauma informed framework.
  • Undertake case planning with our clients and assist them to identify their goals and aspirations.
  • Review case plans, in consultation with our clients, internal and external stakeholders including DCJ, education and health department, on a quarterly basis.
  • Work collaboratively with significant others such as family, carers, educational staff and individual therapists to support our clients.
  • Develop pathways with our clients to make transitions, leave care and case closure.
  • Build and maintain trusting therapeutic relationships with our clients and their families.
  • Drive the process of case planning and permanency planning for our clients.
  • Complete assessments and collect outcome data & maintain key performance as required for reporting and care planning.
  • Work with our clients and their families to identify permanent placements through Family Finding and exploring restoration.

Qualifications:

  • A relevant Bachelor’s degree or a relevant Diploma and working towards a Bachelor’s Degree (Social Work, Psychology or equivalent)

Experience & Skills:

  • 2 years' experiencein case management.
  • Demonstrated experience working with clients, including young people 12-18, who have experienced complex trauma.
  • Demonstrated experience working with the families and young people who have been in contact with the child protection system.
  • Previous experience in residential or foster care contexts and understanding of child development.
  • Experience working with families towards restoration or contact arrangements.
  • Experience working with linguistically and culturally diverse communities.
  • Comprehensive knowledge of child protection and social services practices or policies.
  • The ability to work autonomously and contribute to an effective team environment.
  • High standard of professional ethical behaviour and sound understanding of boundary issues.

Work Requirement:

  • Current First Aid and CPR certificate.
  • Current Working with Children Check.
  • Clear National Criminal History Check.
  • Unrestricted NSW driver’s licence.
  • Eligibility to work in Australia.
  • Provide M180 consent to complete a verification of the Residential Care Workers Register managed by the Office of the Children’s Guardian.
