Job description

Upper Lea Valley Integrated Community Team has an opportunity for a Band 6 Caseload Manager to join the team.

We are seeking a dynamic and motivated nurse with proven community experience to lead and supervise the team, ensuring patients receive high-value healthcare. The ideal candidate will have excellent communication skills and demonstrate collaborative working.

Our service operates from 8am to 10pm, seven days a week. We are looking for an enthusiastic and motivated Caseload Management Nurse to work in the Upper Lea Valley area, providing high-value healthcare to patients in a community setting under the direction of a Deputy Locality Manager.

You will be part of a large integrated community team comprising Band 7, Band 6, Band 5, and Band 3 nurses. Candidates should demonstrate an understanding of current professional and community issues and show commitment and flexibility to adapt to ongoing developments within community nursing services.

The role requires the ability to work autonomously and possess excellent communication skills. In return, we offer professional development opportunities in both clinical and leadership programs.

This position includes a High Cost Area Supplement of 5% of the basic salary, with a minimum payment of £1,192 and a maximum of £2,011 per annum (pro-rata where applicable).

A full valid driving license and access to a car for regular business use are essential unless you have a disability as defined by the Equality Act 2010.

Key Responsibilities:
  1. Provide support to a team of multidisciplinary professionals delivering clinical health and social care in an admission prevention/avoidance service.
  2. Set and maintain standards of care, and coordinate and supervise the nursing team and learners as required.
  3. Assess, plan, implement, and evaluate individualized nursing care for patients in the community.
  4. Maintain accurate records of patient and other relevant data.
  5. Participate in the long-term management of patients within the locality.
  6. Maintain responsibility for a defined caseload.
