Nichols College is looking for a Case Manager - Wraparound Services Facilitator to provide intensive mental health services. This role performs high-fidelity wraparound services based on a trauma-informed and person-driven care model. The facilitator works as part of a treatment team to provide ongoing, intensive targeted wraparound services to YES waiver participants with mental health disabilities, helping them access medical, social, vocational, financial, educational, and other essential services.
The position involves implementing the YES Waiver program, which offers comprehensive home and community-based mental health services to qualifying children, adolescents, and families served by LPS in Collin County. It requires initiative and independent judgment to provide interventions in community, home, and school settings, troubleshooting and organizing support, interventions, and services to achieve positive outcomes.
The ideal candidate should be able to work independently and collaboratively, communicate effectively with supervisory staff, and maintain timely and thorough documentation. Additional expectations include excellent time management, organizational skills, efficient scheduling, managing clinic flow, adhering to crisis procedures, and being available to assist with crisis situations and coverage areas as needed.
